YOU will have the opportunity to submit YOUR questions to him at that Conference via the Questionnaire Form link below. Questions will be asked anonymously.
We must have your form with your questions back by 5 PM Monday, October 14th in order for them to be collated and prepared for the meeting.
Simply follow the directions on the form as to how to submit it.
Questions regarding personal matters are best brought to the meeting so Regional OPWDD Staff will be able to provide the assistance or answers you need for such matters.
